3. $100 Registration Fee - Online
4. Medical Form Download and have your child's doctor complete & sign. Then send a picture
5. Request Immunization records from the doctor.
IMPORTANT: Final step...
Take picture of completed & signed NPR and Medical forms and immunization records received
then click email to send all three to us.
